The procedures need to reflect the intended scope. Motions to approve/modify the agenda, fee waivers, exploratory trips for venues,
and I am sure there are others, in my opinion are not subject to the “motion needs to be on mentor” requirement.

The existing templates don't cover all circumstances, but they do cover more than just forwarding standards. Generally, I would say that the focus is on motions that need supporting documentation.
By the way, I notice that John's latest proposed update covers most the points that I've raised here (though not the part about posting documentation to Mentor or another URL). Two topics of the slide set were:

Item #3 Codify use of Motion Template for electronic ballots

and the last slide proposes to address Items 2 & 3 with an update to the Chair s Guidelines:
2.21 Form of Motions for EC Meetings, Teleconferences, and Electronic Ballots.
It is recommended that the motion template (ec-16-0170-0x-00EC-802-ec-motion-template.pptx) be used for reference for all motions for EC Meetings, Teleconferences, and Electronic Ballots.
The purpose of the adopted motion template (ec-16-0170-0x-00EC-802-ec-motion-template.pptx) is to improve the quality of the work of the EC. Used properly, these motion templates should result in the reduction of motions that are incomplete or ambiguous and the reduction of gratuitous variation. This should save time of all concerned in the preparation and debate of the motions.
It is anticipated that any motion templates adopted by the LMSC EC will serve as recommendations but not requirements. They might be included in the LMSC OM as an annex, or might be a stand-alone document. The existence of a template here does not preclude somebody from presenting an alternative form of motion intended to achieve the same effect. Hopefully EC members will view these templates as a useful tool and use and maintain wherever appropriate.

At the November EC closing meeting, I presented ec-18-0233, entitled "Three Procedural Issues for Discussion." The result was an Action Item: "Marks continue discussions noted in ec-18-0233-00-00EC in rules meetings." In preparation, I would like to raise each of the three issues on the reflector for your further thoughts. This thread covers Issue #3.
Issue #3: Motion Procedures for Ballots
The Recording Secretary maintains procedures for motions at meetings; for example,
*The Motion should be made on a template slide
*Referenced documentation must be posted at Mentor or elsewhere
-Not just as an attachment to an email
I recommend that email ballots follow these same procedures. This could be recorded in the Operations Manual (where 4.1.2 "Voting rules" already covers procedures for email ballots), or in the Chair’s Guidelines.
I would also like to see the Recording Secretary's Procedures for motions, in general, to be upgraded in status, so that they are recorded in either the Chair’s Guidelines or Operations Manual.
I have no specific language to propose, but I'd like to know what others think of these issues.
