The form will not work properly in OpenOffice.
The new form requires custom validations in Excel to function properly. It
looks like OpenOffice does not support the cusom validations, but I don't know
that much about Open Office.
We could provide a version without the data validation for use in other
spreadsheet applications, but I'm not sure we can provide a single file that
performs all of the features in any application.
I have attached a sample of the new spreadsheet.

My personal belief is that the requirement is for an Excel spreadsheet
to work on popular and/or freely available platforms.
I don't envision this to be any different than the need to test web
pages on several browsers.
Open Office does read and write Excel format spreadsheets.
Hopefully you haven't done anything within Excel that the Open Office
folks have not provided for.
I would also be happy to take a pre-release look at the spreadsheet and
see if I can help define any obvious problems. The best solution (of
course) would be to have an Excel formatted form that works well on
either platform so that we do not have to burden the users with the
concern of which platform they well use.
